Filling out form ds-230

i was checking out this http://travel.state..../info_3830.html and found this:

Many U.S. Embassies and Consulates have developed information and guidance that will assist you in collecting the documentation needed by the NVC. To determine if your embassy/consulate has developed any location-specific assistance:

Select the first three letters of your NVC case number from the following dropdown list.

Click on US Embassy/Consulate-specific information.

do i need to fill out that form? if yes where i need to send it:

According to the PDF, it looks like you have to submit that form along with the DS-230/IV package.

I guess that form is for Iranians as there are only two languages in the second page (1st English and 2nd Persian). Called NVC and asked about it from the operator. She was shocked and told me that she has not seen anything like that form before and in fact it says "PLEASE COMPLETE AND RETURN WITH FORM OF-230 (i)." She also said that we can always take it to embassy at the time of interview if it is required.
