How to email the DS 3032 for a minor child??

Preparing to send documents for NVC. My wife's minor son has a separate I-130; the email for her and her ds3032 is pretty straight forward, following the Wiki guide for the DS 3032.

However, his mother will be signing all documents/visa, for him, since he isn't even doing so well with crayons yet, and is just getting Momma and Dadda down pretty good !! So has anyone recently submitted the ds 3032

for a minor child for IR-1/CR-1 petiton??

I've made a rough draft, Subject: his Case number- Beneficiary Name, inserting her name and case number, and that she is his mother of this Case Number (Son)....blad blah blah blah....

Any experience with this, or recommendations? We're going thru Manila..Philippines...

Hey Thomas, yes, I used the template...however, since she is signing all documents, its a little different than just for ONE beneficiary, and its a different person granting the agent. I'll forward you the draft.

We are not petitioning her son at this time because he is school age now, but no English yet.

The way we did the DS-3032 was print off the form, fill it out, sign it, and email back to NVC....so simple.
