Which Medical Form?

For a K1 medical examination that will be performed by Dr. Anga, which form do I need I-693 or DS2054 and do I need to provide it or will the good Doctor have copies on hand?

Additionally, I would like to email with someone who has specifically gone through the process and received an approval via Lagos.

you do not need to take any forms with you, the beautiful Dr Anga has everything you need, you just go with two passport photographs, your passport and your medical fee

Notification Letter

Immunization Record plus 2 copies (Is this optional, not sure that my SO has these records, will the good Doctor immjnize and provide records?)

Totally confused about the fees schedule, I heard something about N12,000 but online searches yields other amount.

How do I know the account number/depositor name and I am assuming I can do thsi at any Zenith Bank?

Are vaccinations totally optional? If so I can get all those done for free here in the US. For example does the Embassy need to see some sort of record?

information about acct and all that should be in the interview packet when u receive.

If you have a shot recoed and up to date on shots bring that to medical. But vaccines are not option there are 3 that are required most get it since not many people keep their shot records.

No the proof of being current on all shots is mandatory and part of ur medical. Depending on ur medical more shots may be required.

Check the USCIS website for shots that are required. Also TB test, blood work, xrays.

Timeline

In addition to the above correct advice make sure she will get a copy of the DS3025 that will list her vaccs. The original will go with the brown envelope.

If the vaccs are incomplete she will have to go to a civil surgeon here in the US to get the remaining vaccs and get them transcribed to the I-693 which you need for AOS or you will get a RFE.

Many panel physicians ( embassy ) will not give these copies unless the beneficiary asks, even demands a copy, though it states to give the beneficiary a copy.

It can get very confusing to decipher if the vaccs are complete or not just looking at the DS3025.

Many civil surgeons here will insist on another expensive complete medical here when the beneficiary only needs to complete some vaccs and it only needs to be transcribed for a reasonable cost of $20-$50.

RFE response : June 26, 2015 returned with a boat load full of financial evidence.

UPDATE: July 5, 2015 updated on all 3 cases, RFE received June 30, 2015.

Service request : Aug 12, 2015, letter received that it will be processed within 90 days from receipt of RFE.

UPDATE: Aug 24, 2015, EAD card being produced/ordered. ( 102 days from AOS receipt day and 55 days from RFE response received.) Thank you Jesus !

Waiting to skip ROC and get 10 yr GC due to over 2 year while pending AOS
AOS APPROVED Oct. 4, 2017 * Green card in hand Oct 13, 2017 !!!!!

First K1 denied after 16 month of AP. Refiled. We are a couple since 2009. Not a sprint but a matter of endurance.

Question of my own....Going back for a second interview, we know he has already had his shots, I know they are also documented in his yellow card. If they did not return this form to him and he has not a copy of it 1. Will he need to redo his vaccinations or 2. Will the Consulate honor the first set of paperwork if they kept it? Seems silly to me to have to pay to redo them, but I guess we will if need be. I know he has to redo the physical, and police report, but I really dont know about this part of it. Plus they switched medical facilities, and the one from last year is no longer on the list. Thanks in advance :)

If the second panel physician accepts his yellow card and verbal statement that he has already had this and that vaccine, then he will note it in the form as complete.

He can also do a titer test to determine which vaccs he has had and which he may still need. Often not all shots are completed but started.

It is up to the new panel physician's discretion on what he feels is necessary and medically appropriate. To randomly repeat all vaccs just because he has another interview is not needed and not healthy.

Good to know, I'm just trying to be proactive since it's shaping up to the possibility that I may not be able to be on the ground with the beneficiary.
