Questions about #2 to #5 (Should I suppose to fill this information i mean this information is about visa that i dont know when they will issue )

#2)Place of Issuing

#3) Issuance country

#4)Issuance Date

#5) Expiration Date

How should i know issuance and expiration date of visa ?

One more question, after we receive Packet 3, should we have to send some forms immediately or all the stuff together (New Delhi Embassy) ?

They are not about a visa. They are about your passport.
