Timeline

Do we or do we not need to go to a civil surgeon? Do we need to still do this form if she is already up to date on all her vaccinations...one part of the guide says no, only if she is missing some immunizations while another part of a guide says yes you still need to fill it out and take stuff to a civil surgeon to sign and put in an envelope. Which is it? Thanks everyone

She has the DS-3025 with a sealed enclosed cd. Do we send that or just the copy of 3025 also the box is checked incomplete with the lower box stating "Applicant may be eligible for blanket waiver because vaccination not medically appropriate as indicated above" Most of the check marks above are for not age appropriate and one check for time interval....So ummm not sure and I'm sure its some simple answer but my mind is in overload with everything else.

Ok thanks....read through it so my understanding is that we do not need to file the I-693 as we have that paperwork so we will cancel our appointment with the civil surgeon....thank you
