Vaccination Supplement

Dear Friends,

This might be a silly thing to ask, but I am so confused. I had a K1 visa from India and at the time of the medical examination, I asked the doctor if I needed the vaccinations. She said that its only required if I intend to study in the US. So I did not get any vaccinations at that point. Came to US, got married. Now my question is, I have been reading about this vaccination supplement thing. Wat is it? And where can I get it done? What are the DS 3025 and I 693 and how are they related and which one will I need to file for the AOS?

You will need to attach the "Vaccination Supplement" to your AOS application.

You can get one prepared at a Civil Surgeon that is approved by USCIS in your area. If you received a "Vaccination Worksheet" from your K-1 Visa medical exam, you can take that with you and they can transfer the information from the worksheet onto the Supplement. If you require any necessary vaccinations, the Civil Surgeon can also give those to you and then document them on the Supplement.

The immunizations are not just necessary if you intend to study, they are necessary for every immigrant.

Call around to lots of Civil Surgeons to get prices, because they vary greatly, and some will try to tell you that you will need the medical exam again in order to get the Supplement. If your K-1 Visa medical exam was less than 1 year from the date of your AOS application being submitted, then you do not require another medical exam.

Our Civil Surgeon charged us $35.00 for the Supplement, but no vaccinations were required.

Hy Vipul, didn't you get a vaccination worksheet form the doctor in India? If you didn't then be prepared to get a bunch of vaccines and pay for it all also. Good Luck
