Hello all, after a couple of months of reading this forums posts, I am pretty much ready to ship out my AoS package and just wanted to see if anyone could give my contents the once over to make sure I don't miss something by mistake.

Quick rundown of my situation: UK Citizen married whilst on an F1 Visa to USC. Visa has now expired and I am applying for my adjust of status.

My AoS package contains:

Money Orders for $1490 made out to U.S. Department of Homeland Security

G-1145

I-485

- 2 x Photos of myself

- G-325A

- Copy of Birth Certificate

- Copy of I-94 Front and back

- Copy of my Passport Visa page

- I-693 and sealed envelope from medical center

I-131

- 2 x Photos of myself

- Copy of my Passport

- Copy of my Passport Visa page

- Copy of I-94 Front and back

I-765

- 2 x Photos of myself

- Copy of my Passport Visa page

- Copy of I-94 Front and back

I-130

- Photo of myself

- Photo of my wife

- G-325A completed by myself

- G-325A completed by my wife

- Copy of my wifes US Birth Certificate

- Copy of my wifes US Passport

- Copy of our apartments lease agreement

I-864 (Wife)

- Last 3 years 1040 forms - Zero income unemployed

- Copy of US Passport

My wife has been unemployed for the last 3 years, she recently submitted the last 3 years tax returns with zero on everything so she could include them with this I-864 form. A joint sponsor will be making up the financial requirements.

I-864 (Joint Sponsor)

- Copy of US Passport

- Last pay slip (June 2012)

- 2009 Tax Return Transcript

- 2010 Tax Return Transcript

- 2011 Tax Return Transcript

Joint sponsor makes over 125%....

I think that should be everything! If anyone spots anything a little off I would really appreciate your advice as I want to get this all correct the first time.

Another thing I was unclear of was whether I need to include a copy of my I-20 with any of these forms?

Being in California can someone confirm the address of the Lock Box I should send my recorded delivery AoS package to? The I-485 instructions say as I am using USPS Express Mail I need to send it to:

USCIS

1820 E. Skyharbor Circle S

Suite 100

Phoenix, AZ 85034

is that address correct?

The last thing I am unsure of is if my J1 Internship visa I obtained 4 years ago qualifies as me "Applying for employment Authorization from USCIS" and I need to select Yes in section 11 of the I-765 Form where it asks if i've applied for employment authorization before? Then in turn supply my old J1 documentation.

I am currently on an expired F1 student visa, but 4 years ago I came out on a J1 I then completed a change of status to F1 then went home to the UK and reapplied for the F1 again.

If someone could advise if the J1 counts as employment authorization that would be great. I have a feeling it does as I paid some Federal tax and was also given a SSN.

Thanks in advance guys!

I have suggested this be moved to AOS (Other) as that's the proper place for this.

Also no. AOS is sent to Chicago.

I recommend you read your form instructions carefully and check each form on its own to make sure that each form has its relevant evidence attached.

you will also find that if you are filing your I-130 and I-485 concurrently, you are sending your packet to the Chicago Lockbox, as outlined on the I-485 instructions. Both Express/Courier and USPS addresses are bolded and boxed.
