Hello everyone,can anybody help/advice us? In the I-765, Application for Employment Authorization,there is a portion there,which USCIS office? Date. Are we the one who choose for a date and USCIS office? In I-693,report of medical examination and vaccination record,i believe that i only need to transfer my vaccination record to that form through a USCIS civil surgeon. Can anybody tell me if which specific vaccination needed in AOS. i got a complete series of shot in MMR,Tdap,HepA-HepB and i had a history of chicken fox so i did'nt get the Varicella Vaccine.I did'nt have a flu vaccine since its not a flu season. We are trying to know the specific vaccination requirements since all the USCIS civil surgeon which is near in our place and neighboring town told us to pay $300 just for validating the I-693 form which is too expensive. Replies are greatly appreciated.THANK YOU!

If all your vaccinations were complete at the time of your medical and marked off on your DS-3025, then no need to see a civil surgeon. If your DS-3025 is marked incomplete and may be eligible for a blanket waiver, but you still had the MMR & Tdap and varicella marked with VH for chickenpox history, then you most likely still do not need to see a civil surgeon. on the line which asked which USCIS office, my answer was "none" and no entry for the date. cause it was related to the question on question 11 asking whether or not i have applied AED before which i answered "no" since this is my first time to apply.

the form you're talking to as "form through a USCIS civil surgeon" is the I-693 itself. some civil surgeon would recommend you to undergo that medical exam and several vaccine shots. with my experience, i printed the I-693 form instruction highlighting the part that for those who enter the US with a K-1 visa had our medical in our country of origin. and we don't need to have another medical exam as long as the medical is less than 1 year. as for the vaccine i talked my way out of that as we have no budget for it.i for one, would recommend it as it is good for us to protect us from diseases through immunization. if you have the budget for that, go for it. if you don't have, talk your way out of that.if they insist then shop around who would allow you to skip those shots. if you would, drive further like 2-3 hours. you could make it a road trip for you two and have fun while on it.
