Advice in getting an NBI clearance w/ AKA

I know it has been discussed on some threads already but since I haven't read about where to get an NBI clearance that has to have an AKA, I opted to go to a satellite branch near our area and I got nothing... They told that they don't print clearances w/ AKA's. For what reason? Well, they just told me " SOrry we don't have that here, we can't do it, you have to go to the NBI main office." Duh?! I waited in line for like 3 hrs and get disappointed. So to those who needs to have AKA on their NBI clearance, GO TO THE MAIN OFFICE. Its much better to be sure than take chance and be sorry. Main office is quite nice anyway... And don't forget to tell the typist on the 2nd floor your concern regarding the AKA. Most of them tend to IGNORE it despite of it being written on the form. When I was getting mine, the typist skipped the space for the AKA and when I told her that I need my previous name written on the AKA, she gets annoyed. But I remained polite, i don't wanna act RUDE, I don't wanna be LIKE HER! :rolleyes: So anyways, she did as I told her though she gives me that "annoyed look", but the hell I care, I don't need her to like me, I just need my clearance! hehehe.. :rolleyes: So then, I was fortunate enough to get through that stage but not lucky enough to get my NBI clearance on that same day. It is to be released on Friday. I hope this will be the last time i'll get an NBI clearance. :P So to those who are in the same situation as mine, good luck!!

And oh, before I forget.. Wearing of slippers are not allowed in the main office but sandals are okay. Don't believe the shoe vendors outside the office if they tell you you wont be able to enter the premises if you're wearing sandals. they just want you to buy shoes from them.. Again, slippers are not allowed BUT SANDALS ARE OKAY. You might want to dress appropriately so you wont have any problems. Ciao! ;)

Request in your current name and then AKA for all other names used

Hi VJ

how can I fill up the form since I am annulled. Should I put renewal since I have old and in my married name? And fill up for single name?

What should I put in AKA? Please help.. I am confused ..
