Sample Letter for Name Corrections

Hey all,

I recently contacted the Manila Embassy for some assistance on correcting the name entry for my fiance's daughter (her last name is her biological father's and wasn't listed properly on the approved I129F), and am having some difficulty following the instructions that were provided by the otherwise difficult-to-understand call centre (not her fault, just a crappy phone connection):

What it boils down to is that she wanted me to submit a 'Name Change' by following the instructions listed at the following link (http://manila.usembassy.gov/wwwh3236.html). What it basically states is to include a copy of the birth certificate and passport for this to be corrected.

With that in mind, I had a few questions for people who may have gone through this:

1) Does anyone have a sample letter I can use as a guide for submitting a name change to the embassy, and would I have to send it or could the beneficiary in Manila do this instead?

2) Since her daughter is illegitimate and the instructions are unclear, should my beneficiary also include her passport and birth certificate in the correction to verify she is the mother of the child?

3) On the instructions for the Interview (http://manila.usembassy.gov/wwwh3238.html), there is information stating more or less if a child is not listed, she would need to provide a notarized letter from me saying that I'm aware of the discrepancy and am still wanting to proceed with the marriage. Would this change of the name on the form (the rest of her documents are correct) be a situation where I should send one of these as well? I was going to be mailing her all the supporting documents on Monday and if I could save money by sending it to her at the same time.

Any help that could be provided would be greatly appreciated. Thanks again!
