I hope somebody can help with this. We are trying to prepare all the papers for adjustment of status. Are we going to use the family name of our husband this time? One more thing,are we going to book an appointment to the doctor(civil surgeon) and send the result together the adjustment of status application,or wait until we have an interview?Last,i can't find a downloadable form that the doctor can fill up,is it available at the doctors office? I read somewhere here that we will just fill up some portion on the form then the doctor will fill up the vaccination portion and give us the sealed envelope is that correct?

You only need the civil surgeon if the vaccination sheet was not signed as complete. If it was complete and your medical is still within 1 year, you do not need the the civil surgeon. The civil surgeon gave us the sheet to fill out. He faxed one ahead of time, then he realized a new form came out, so we filled out the new form while we were there.

Use your married name as you want it to appear on your EAD/AP and green card. Using your maiden name can be a bit of a mess, depending on how your state handles name changes after marriage. But using your maiden name for the AOS can make it darn near impossible to get your married name on anything afterwards.

If you are taking your husband's name after marriage, then use that name on all the AOS forms. The name you use on the forms is the name your EAD/AP and green cards will come in.
