Because your entry has been within the year of doing your medical overseas, the only thing you will need is the I 693a from the civil surgeon.. if you have a copy of your vaccinations from overseas they should just be able to copy it over.. this cost varies so make calls around your area to see who charges the least.

Personally I think it is stupid that they make us do this even if we have had the full immunisation... If a person hasnt had the full range of shots thats a different story but doing it to pretty much all is just another waste of cash.

So now doing my research, I figure out we need the DS-3025, and we don't have it. Do you all think a CS would fill out the i-693a from german vacination records? Same difference in the end really.

Thanks

Scott

You can call the doctor's office in Frankfurt and ask them to fax or send the DS- 3025 to you, I've heard that they are very friendly and helpful.

I've also heard that there are CS, who are willing to to copy your German vaccination records (if international vaccination book") to the I-693a. You just would have to do lots of phonecalls to find the right CS, because most of them try to talk you into completing a new medical, which is of course not necessary, if you are in the 1-year timeframe.

I only had my German Shot Record (it was even the very old east German one) and I called the doctor, I asked if he would do it from German into English and he asked me to help him with it. The words for the vaccinations are often the same and he had it all right except for one or two but that really wasn't a big deal. Usually - from what I have heard - foreign doctors are agreeing more often on doing this than american doctors are (mine was from India and I coulnd't even pronouce his last name).

Are you going to any CS or does it have to be one from the USCIS list?

It has to be a USCIS registered CS. You can't just go to any doc for the I639a vaccination supplement.

Well I don't know if that's gonna work, but you can try. i was in the same case. What I did is that I gave a phone call to the doctor who performed my examination and I asked him to mail a copy of my DS 3025. Which he did. Why don't you give a call to your doctor in Frankfurt?
