Timeline

These are probably really obvious questions, but some easy answers are appreciated.

I previously filed for a SSN shortly after arriving in the USA, but they sent my application to Homeland Security to be verified. I called and called about it, they couldn't tell me anything and I was eventually told my application had expired, which I didn't know could happen but oh well. Anyways, now that I have my EAD, I am filing again for a SSN but was wondering about part 1 and part 11/12.

Part 1, when it asks for other names used, should I put my maiden name?

Part 11, it asks if I ever filed for or received SSC before. Yes, I filed.

But then, part 12, it asks for information that was on the card, which I never received (never received a card or number.) Should I mark the box yes for filing, but leave part 12 blank? I just don't want to mistakenly lie or something.

Just take the form to the SSA and fill it out there. There, you'll find a whole office of people who know their own definitions (such as , whether or not what you did counts as applying for a SSN or not).
