Medical form i-693

HI VJ!

I am trying to gather up information and documents to send with form i-495 "Adjustment of Status" for my wife. I have a question regarding the report of medical and vaccination record form (i-693). My fiancee already had a medical exam in China and the doctors told her all she needed was additional vaccination when she got to the states. My question is, what is the i-693 form asking? How do I fill out the form? Does she have to go for a additional medical exam? My fiancee (wife) came here on a k-1 visa and she is up to date with the vaccination.

If her vaccination report says "Vaccine History incomplete" then she should find a civil surgeon to have the vaccination DS 3025 completed and transcribed over the I-693.

If her medical is within 1 year validity date then she doesn't need another medical examination but she needs to see the CS regarding the vaccination tho.

find I-693 form online. Download copy and fill in form. make appointment with approved doctor found on the list via the online system. doctor will do an assesment at which time you can provide documentation and proof of doctor's visit which is usually required to get a K-1 visa. if not some additional fees beyond the doctor's for the shots you need. option to choose religious or moral exemption, just speak with doctor. They will prvode you with a sealed envelope with the finished form that you will send in. It is all on the directions for the form found here http://www.uscis.gov/i-693
