Hi to all, I hope everyone is having or are going to have a great 4th. I have a question, I am ready to file for AOS for my wife ( from Peru, if this matters ) and want more info on the form 3026. We ( she ) never received this. When was she supposed to get it and what can I do about it if we do not have 1? Was she supposed to receive it from CBP or the Dr. that examined her? Thanks in advance for all your input. We do have a form from the Dr. that has her immunizations on it.

I think you mean the DS-3025. A copy of it should have been given to you by the panel physician that did the medical in your country. If you don't have a copy, and cannot get one if you contact the clinic that performed your medical, then you will need to take any vaccination records you have to a civil surgeon. If you have none, you can simply get the vaccinations required for AOS: MMR, Tdap or TD, varicella(unless you had chickenpox already), or pay to have titer tests done to see which ones you need and don't. You can get your vaccinations at the local health department, most likely cheaper than the civil surgeon will charge for them, and take records from the health dept. that shows you had those vaccines to the civil surgeon to transcribe onto the I-693.

You are not required to get a new medical exam. You need to call around to find a civil surgeon that won't insist you need another medical. It can get frustrating, and you might have to call some that are a farther drive from where you live to find one. You can print out the instructions for the I-485 and I-693 to show them. When you call, make sure to explain clearly that someone adjusting status from a K-1 visa does not need another medical exam if theirs was done within the year. Make sure you tell them it states that in the form instructions and you would be happy to print a copy to show them.
