DS-156k Question

Hey guys, filling out some Packet 3 Fiance visa paper work for the Montreal Consulate and I came across something in the DS-156K form http://www.state.gov/documents/organization/82063.pdf

In the middle of the form, it asks for attachments, I've read online plenty and in regards to Packet 3 I haven't seen anyone mention to attach what this form is asking for. I believe this stuff is only for the interview? But why would they place it there? I found an answer online however it wasn't for Canada, so I want to make sure in Canada these are things only for the interview as well.

I'm assuming these will be attached to this form at my interview?

Also, I never received my packet 3, it's been over three weeks. The information here is recent and all I need right?

Also, for the DS 160 form it states "Electronically submitting your DS-160 online application is only the first step in the visa application process. Once you have electronically submitted your DS-160 online application, you must contact the embassy or consulate at which you wish to apply to confirm whether you need to be interviewed by a consular officer, and to schedule an interview. You can find a list of U.S. embassies and consulates here, with links to their websites where you can find information about scheduling a visa interview appointment. If the embassy or consulate at which you apply informs that you must have a visa interview, the visa application process cannot be completed until you appear for an interview with a consular officer."

Do those applying for K-1 fiance visa have to do this? Packet three just asks for a receipt page

You should try not to start more than one thread. Check your other thread for more answers.

You do not have to receive packet 3 in the mail before you send it back. Call department of state and ask if packet 3 has been logged (you need your MTL case number for this). As soon as it's been logged you can mail it back- the hardcopy is basically a printout of what you found online at the Montreal consulate website. It is common for packet 3 to take a long time to arrive in the mail or to never show up at all so it's best not to wait around for it.

Yes you have to fill out the DS 160 (the confirmation receipt for it is required in packet 3). Also if possible print out the form for your own records (there should be an option to do so but they might have changed it in the year since I filled out packet 3 myself). But you don't have to contact Montreal after filling it out.

All you need to send with packet 3 is this:

If you are a Fiancé (K-1) applicant:

1. DS-230 part I only – one copy per each applicant, including children

2. DS-156 – 2 copies https://evisaforms.state.gov/ds156.asp

3. DS-156K – 1 copy http://www.state.gov/documents/organization/82063.pdf

4. Confirmation receipt regarding Form DS-160 - one copy, available online at https://ceac.state.gov/genniv/

5. Below checklist of documents to gather for the interview – one copy per family is sufficient.

6. 2 color photographs (Photograph Requirements)

7. Photocopy of the biographic data page of the applicant’s passport
