You can read vaccination requirements here: http://ukraine.usembassy.gov/iv-medical.html

She can try to do them from her local doctor which should be pretty cheap, and when he goes to the medical, just give it there and ask them to fill out DS-3025.

Usually people get most of their vaccination done in childhood in Ukraine. So she should have no problems.

The vaccination is required for AOS regardless of country. However as I stated she can get them done in Ukraine which is much cheaper, just make sure that during medical they will transfer it to DS-3025, and if they will say you are missing one, just ask them to do it.

What I've seen based on my experience since my wife had her vaccination in childhood, the one they might tell you to do is: Rubella. As seems like it is usually not done in Ukraine. But other then that you should be good. You will save a lot of money going this route.

Tatiana had all hers in Ukraine, much cheaper, and made sure they were transfered on 3025, like they said. When we got to states they said also required was chicken pox. Don't know if that is just state requirement. I got my local doctor to get it. Was much cheaper than having civil surgeon do it. Check different civil surgeons for pricing. We had everything transfered to I63 from the 3025 for $5.

I would suggest getting them done there, instead of here. While it's been a while for us, we could only get them done by a Civil Surgeon and there isn't that many out there and they don't accept medical insurance cards for this type of work. The cost for like one shot for my wife and two for or daughter, plus his initials on a few places & the offical stamp he was a Civil Surgeon, cost us over $500 EACH!! So get as much done as you can before they come here and good luck.
