Hi everyone! We got married June 25 in Maryland state. And as I'm preparing to file for AOS I'm stuck with a couple of issues.

1. Changing name after marriage. How do I do that? I'd like to file all my AOS documents with my new last name. I read somewhere that I have to change my name on my SS card first (I do have one from when I was coming on J-1 visa). Is that right?

2. Medical exam. I got my exam done a month ago while being in Ukraine, but the results of the exam were taken by the officer at the port of entry at Dulles International Airport. So now I'm left only with vaccination report which I took from my doctor in Ukraine. Is there any way to retrieve those medical results or will I need to take the exam once again?

3. Marriage certificate. We've got a marriage certificate from our minister right after the ceremony. But now I've read that we have to obtain a certified one. Can the marriage certificate we've got from the minister be considered as a certified one?

File your papers as you want your name to be. SSN name change is easy after you get GC and marriage certificate. I believe my wife had a packet of medical stuff, contact a civil surgeon in your area for advice. no you need a certified certificate. 11. I-693, Medical Examination of Aliens Seeking Adjustment of Status.I-693, Medical Examination of Aliens Seeking Adjustment of Status. If you are a K-1, K-2, K-3 or K-4 visa holder the I-693 instructions state that you are not required to have another medical examination as long as your Form I-485 is filed within one year of your overseas medical examination. If you are missing any shots from your overseas medical a I-693 vaccination transcription is required to be completed by seeing a Civil Surgeon to have certain portions of the I-693 completed and have the Civil Surgeon certify the form (sealing the original in an envelope and giving you a copy). reread the guides.

You can take your certified marriage certificate to the SSA and have your name changed to your married name. Sometimes they won't do it without you first filing for AOS and getting your EAD or green card. In any case, the name on your SS card does not need to be changed first in order to file for AOS. You want to fill out all the forms with the name you will be using from now on, which is your married name. The marriage certificate is proof of legal name change.

2. Medical exam. I got my exam done a month ago while being in Ukraine, but the results of the exam were taken by the officer at the port of entry at Dulles International Airport. Everything that was in the sealed envelope that was handed over at your POE was sent to the USCIS. They have all your documents, including your medical papers. You need to check if your DS-3025 vaccination sheet is completed in a way that allows you to skip a civil surgeon and just send in a copy of the DS-3025 with your AOS paperwork. The certificate from your minister is just a keepsake for you. Your marriage license needs to be sent to the court to be registered and legal. You can get copies of your certified marriage certificate from the court. If you have not gotten those, then you need to go get them. You need the certified copy from the court to change your name on things and file for AOS.

1. File the AOS under your married name Change the SS card after you get your GC. It's not important that the SS name matches the GC. The GC name is more difficult and costly to change later.

2. The vaccination form you have should be the DS-3025. If it is marked complete then you don't need the I-693. If it is not marked complete then you will need a civil surgeon to complete the necessary vaccinations and fill in the I-693. You don't need another exam if filing AOS within 1 year or the previous exam.

3. You need a certified marriage certificate from the courthouse with the seal stamped on it. The one you have is not certified.

Don't let what you cannot do interfere with what you can do.

You don't need another exam if filing AOS within 1 year or the previous exam.

It can be marked incomplete and say 'may be eligible for blanket waiver', in which case you could possibly still have all the required vaccinations for AOS and not need to see a civil surgeon. It isn't quite as black and white as just complete or incomplete. That is why I posted the link above so she could check to see if her DS-3025 was completed in a way that allows her to skip the civil surgeon. Thank you all for your help! I have a couple of questions left regarding. I forgot to mention one important issue. I got my maiden name indicated on our marriage certificate. Is it still ok to use my new last name on AOS documents since they will see I'm using my husband's name? Or I should have indicated my new name on marriage certificate? (I don't believe we had this option, though).

I know I will have to go to the civil surgeon so he could fill out my vaccination report because I know it is incomplete. But I'm worried because it was written in the guidelines that he also has to fill out the first page of the medical exam (summary of medical examination) where he has to indicate I have no A/B medical condition. So now if my medical has already been sent to USCIS I don't know if I'll be able to get that vaccination report right.

Not all states allow you to list the name you choose to go by after marriage on the marriage license. In that case, the certified marriage certificate from the court acts as a legal name change document. You just show a copy for your social security card, drivers license, IDs, and to change your name on accounts and such. If you will be using your husband's last name from now on, be sure to fill out all the AOS forms with your new married name so your EAD/AP and green card will come in your married name. The forms have a place to list previous names used, and that is where you list your maiden name.

Yes, you have to get any missing vaccinations and have page 1 and the vacation page of the I-693 filled out by a civil surgeon. Take your DS-3025 and any other vaccination records with you so they can transcribe those to the I-693 and know what you are missing. You are not required to get a new medical exam. You need to call around to find a civil surgeon that won't insist you need another medical. It can get frustrating, and you might have to call some that are a farther drive from where you live to find one. You can print out the instructions for the I-485 and I-693 to show them. When you call, make sure to explain clearly that someone adjusting status from a K-1 visa does not need another medical exam if theirs was done within the year. Make sure you tell them it states that in the form instructions and you would be happy to print a copy to show them.

That's the thing they took my DS-3025 at the airport without giving me a copy. So now I guess I'll have to pass a medical all over again...:(

First off, you are not required to get a new medical with or without having a copy of your DS-3025. A copy of it should have been given to you by the panel physician that did the medical in your country. If you don't have a copy, and cannot get one if you contact the clinic that performed your medical, then you will need to take any vaccination records you have to a civil surgeon. If you have none, you can simply get the vaccinations required for AOS: MMR, Tdap or TD, varicella(unless you had chickenpox already), or pay to have titer tests done to see which ones you need and don't. You can get your vaccinations at the local health department, most likely cheaper than the civil surgeon will charge for them, and take records from the health dept. that shows you had those vaccines to the civil surgeon to transcribe onto the I-693.

You will have to search through that post I linked above and call around to find one that will not insist on giving you the medical again. It is a hassle and can get frustrating. Just print out the instructions for the I-693 instructions and let them know a K-1 does not have to get a new medical.
