CFO guidance and counseling form

Timeline

My fiancee's last step before she leaves will be attending the CFO seminar either later this week or sometime next week. I found the link for registration, but cannot find where to download the guidance and counseling form that she needs to fill out.

There are many, many threads here on VJ with cut-and-paste info from the CFO website referencing the form, but no links to the actual form itself. Searching the CFO website yields the same result.

Anyone have the actual link to the form?

We just filled out 2 registration forms which are on the website and that is what she is going with. She hasn't gone yet so not sure that is correct. I figured if there was another form she could fill it out there.

Also a question? Her mother is going so is there any problem with her attending? I figure they want a consent to marry so might as well put mom and the signed document in their face..

16 July 2012: Manila Case Number(by phone)

17 July 2012: Interview paid at BPI

24 July 2012: CFO Seminar(had to go next morning for landline #)- PASSED

02 Aug 2012: Received e-mail from USEM our case is there.

15 Aug 2012: Interview at USEM - APPROVED

13 SEP 2012: POE Minneapolis, MN

27 OCT 2012: Married

19 NOV 2012: AOS package sent

05 DEC 2012: NOA's I-765, I-131, I-485

14 DEC 2012: Biometrics appointment finished(Walk-in..Was scheduled Jan 04 2013)

07 FEB 2013: USPS Picked up the combo-card

11 FEB 2013: Received Combo-card

21 FEB 2013: Transit Visa picked up in Chicago for Japan

Thats one the one we filled out. But I undertand the question that was mentioned about the guidance and counceling form they mention on the website, i just assumed it was the registration form that caryh just supplied..

She can get a form in the counseling and fill-out there..

Here's my understanding about that form. Although the CFO is only required for filipinos leaving the Philippines, the guidance and counseling form is required for anyone under the age of 25 to be married. Your fiance and her parents can go to the local registrar's office and complete the form. I've seen the form on a couple of registrar's websites at different times. It really should be signed by both parents, if they are alive/in the picture. So having the mom go to the CFO can help, but not really a silver bullet. It is realy up to whoever is working at the CFO that day, as to what they will accept. Some days seem to be easier than others. The local registrar (where she would go apply for a marriage license if you were marrying in the Philippines) should have that form there.

Ohhhh... so if this form is ONLY for people 25 and under, it's not applicable in our case because my fiancee is well over that age and she won't need the form.

Straightshooter, to answer your question, you said you had the registration form already, which you will need, but given that your fiancee is over 25 you won't need the guidance and counseling form, which is what you were asking about.

On my experience, We went to the US embassy at around 12noon to get my visa, we decided we don't want a courier since we are just in the bay area. We ate lunch, then rode a taxi to PRISM and before the end of the day I'm done. They are kind of tired towards the end of the day, so they didn't ask so many questions. During the seminar they just focus on the main things we should know. I'm back to the hotel at around 5:30. For me it worked better to come in the afternoon.

You just have to have some loose change ,cause I remember I need to photocopy something ,

You can complete the seminar anytime before or after the interview. However, you won't be able to get the CFO sticker affixed to your passport until you have your visa.

My fiance and new daughter (7) will be going as soon as they get there visa and passport back, my question is do i have to pay for a 7 year old child being she doesn't need the counseling? or do i have to do everything the same as my fiance will do? Thanks
