Form I-134

My I-129F was submitted and received and now I am filling out the I-134. I read that this may not be needed but I will send this to my fiance just in case. At the bottom of the I-134 (Rev. 05/25/11) in the signature block in the end of the Oath or Affirmation of Sponsor, the end of the first sentence: ...Food Stamp Act, as amended.__???__ I certify under penalty..... There is block to fill in with either a date, number, signature, or my printed name. I looked at the example form here on the forum, which is a great forum by the way, but the example form shown is from 2008 and that Rev. does not have this block to fill in.

Any thoughts?

It is just an out of place line. You do not need to fill anything in there. Just sign and date where indicated.
