OK so guys my husbands first medical is Monday the 25th and i had a question the papers that come with the medical instructions there is a first page and last page that needs to be filled out so we are freaking out because we don't know if we should fill it out or do they fill it out at the medical clinic when he goes there... help plzzzzz

why dont you guys call the doctors office and just ask what you should do? or call the embassy? thats the best route for you, :thumbs:
