Does anyone have/know sample of late registration of marriage affidavit form? Need it so badly...please help anyone...thank you.

Nov. 06, 2008 - VISA ON HAND...Thanks GOD, long wait is over

Nov. 12, 2008 - Arrived @ JFK Airport 3:30pm (US time) POE- Detroit

Does anyone have/know sample of late registration of marriage affidavit form? Need it so badly...please help anyone...thank you.

Here's a sample of an Affidavit of Late Registration of Marriage. You would need to edit some things (ie city, country), but at least this gives you an idea of what to include.

Please refer to the Report of Marriage Contracted Abroad from the New York Philippine Consulate as your guide.
