Help with DS-156

Hi everyone,

I'm hoping some of you knowledgeable people out there will be able to help me with this! I'm working on completing form DS-156, and am stuck on question 30, which asks about previous visas. I had a student visa (F1) from 2005-2008, followed by "optional practical training" from 2008-2009. I no longer have the actual visa, as I had to turn in the old passport when I renewed. All I have are a I-797C Notice of Action, a bunch of I-20's, and a copy of my I-94. Here are my questions:

1) Is the Optional Practical Training considered a seperate visa, or is this just a part of the original F1?

2) Where would I find the date issued, having only the I-797C NOA, the I-20's, and the I-94? Is it the notice date from the I-797C NOA?

3) Question 30 asks for where the visa was issued- are they looking for a service center, a POE, or something else? If it is a service center, how can I find out which one mine was processed at?
