Medical Form + Evidence question

My first question is the Vacination Worksheet the only form they need from my fiance's medical? What about the xray of his lungs?

Second question is about our marriage. We got married on May 25th and I am sending in the certificate. Do they want pictures of the wedding? Contracts + payment from the banquet hall etc?? Or is this not what they need? :help:

I sent copies of all that the doctor gave me, so yes, the x-ray certificate too (not the actual x-ray).

And just your marriage certificate, nothing else is needed.

The only thing he had for the xray was the xray itself, no certificate or anything. So I sent that as well. I guess we will see what they said... hopefully everything will be okay, I sent in my packet today.

If your spouse had all the required vaccinations at the time of their medical, then all you need to send in is a copy of their DS-3025. They don't want the chest x-ray. That is yours to keep.

The only evidence of marriage you need to send is a copy of your certified marriage certificate.
