We just got approved in the USCIS, and I am looking into the required documents for the NVC. It appears a copy of the beneficarys passport biographics page is required. When we got married she decided to take my last name and has changed her name on all her records. She has not yet changed her name in her passport, and has her old name in it. Am I able to still use this?

her issuing agency (passport) should be able to make a "page 4" endorsement. aka (also known as) = and put her married name here. they should stamp/seal/sign or in some way validate the alteration to her passport. there should be some equivalent in your wifes passport i would imagine.

we did this for my wife. her passport data page has "maiden" name. page 4 has her married name. we included a copy of page 4 and the data page during our submission to uscis and nvc. it also allows my wife to use her passport with her married name rather than having to re-issue a new passport for one that is still not expired

Also, I am a bit confused. Does the passport scan need to be given to the NVC or the NZ consulate at a later date? I have also seen conflicting answers about the police report. One page says it needs to be sent to the NVC, then another says the consulate will mail you a authorization form. Do we need to fill out the authorization form for the police reports now for the NVC or later for the consulate.

Yeah, I would have started sooner if I realized this was needed. I thought they took a copy during the interview. I must have misread or forgot. Luckily, standard passport processing in NZ only takes 10 business days. She can send a scan electronically, so that's not a huge deal. The biggest deal is the birth cert. We don't want to give up her original birth certificate, so we have to wait 10 days for one to be mailed to her house. Then she has to have it shipped over to me so I can put the packet all together for her.
