does everyone need to complete this form? i am going over the guide but do not still understand it. please help

every one that comes to the united states has to have medical examination,the medical form, 1-693, medical examination and vaccination record.for the interview

question is if u are sending out the AOS, do u have to send that form with it?, or is part of the brown envelope they received at the airport?

YOu have to have it filled out by a civil surgeon who will transcribe the vaccines from the card that was provided at your ghana medical. You do not need a medical if you came on a k-1 and your medical is not more then a year old.

So you don't fill it out- the Dr. Does....
