Hello All,

My wife got her K-1 visa in March 2012, and came in May 2012 , where we got married.

This I-693 form is kind of confusing, and I am lost here.

My wife states that she did had her medical examination (blood test, chest xray etc etc) prior to her visa interview in Pakistan, but the medical office never gave her any report for her records and just handed sealed envelope that she took it to the embassy for her interview).

Now, my question is - what part of I-693 is required to be filled by the civil surgeon. She does not have a vaccination record (DS 3025), but we are willing to pay $$$ for the vacinations here. As per the instructions for I-693, a K visa holder only needs to submit part 1 and the vaccination report filled by the civil surgeon. If we are only to do the vaccinations, will USCIS require proof or documentation asking for her overseas medical examination (becuase we dont have it)...

And secondly, does part 1 and vaccination form need to be sealed , or does that only apply to a full medical examination....

I will sincerely appreciate if any of you could advise....

The 693 form is to be filled out by a Civil Surgeon, Contact your health provider if you have one and talk to them and explain to them what your needs are. I recall doing this many years ago when we became citizens. The Civil Surgeon requested to review the medical vaccination records we had from pakistan, the ones we had were in english and Urdu. So he accpeted them as we had given to them, I dont konw if they were varified. DO you have any Vaccination records?

BTW congratulation on the transission from Fiance to Husband. I think im behind you by 6 months! Lets hope all goes well!

You are not required to get another medical exam. You need to have Page 1 and 5 of the I-693 filled out by a civil surgeon. If you have any vaccination records, then take those with you to your appointment. For AOS you are required at least 1 vaccination of MMR, 1 Tdap or TD and varicella, unless you already had the chickenpox. The flu vaccine is only required during flu season, so you won't need one of those at this time of year.

You may have to call around to several civil surgeons till you find one that won't insist on giving another medical, and you might need to print out the instructions for the I-485 and I-693 to show them a K-1 does not need the medical if they had one within a year already.

Same thing happen to my fiance too when she came here from pakistan. She had no medical documents with her when she arrived here, because the embassy took them and then at the airport POE, the immigration officer took the cd she was given. Anyways, went to the local civil surgeon doctor,(you can find one closest to you, by putting your zipcode at the uscis website and it will give you long list of doctors who will do the i-693). The doctor we went to told us that if we have any records of any shots done, he can simply put them on the form and say we have done that, but we had no records of anykind. So we had to do all the shots, a tb test, and blood test. it cost around $400 to do all that. Once that was complete, he filled out the whole form, check the boxes that said each shot was done, put it in a envlope and sealed it with his stamp and we send that with our AOS application. So the thing is , if you have any kind of record that shows you have done your chickenpox shot, the MMR shots, and blood test, the doctor can simply transfer those records on i-693 and away you go, will cost alot less. NOw i have heard that if you have done your medical in last year you dont have to do i-1693 again, but we did it anyway just to be safe.

If u had ur medical done from agha khan hospital they keep the record of DS3025 and hand over 1 photocopy to applicant as well the orignal copy is sent with the sealed which you have submitted in embassy. If by chance you have not been given DS 3025 then ask them for a copy. If u not had ur medical from agha khan then in that case DS3025 must have been provided by the hospital in that sealed envelop which u have forwarded to embassy which means u can still file AOS without medical form becoz the original DS3025 is within that sealed envelop for USCIS. Now lets assume that u dnt have anything to show ur medical record to USCIS. A k-1 visa applicant is exempted from I-693 ONLY and ONLY if DS3025 has been marked as COMPLETE by the panel physician. I get my 1 shot from agha khan and my DS 3025 is marked as complete i also had 1 copy which i will forward to USICS at the time of AOS. In ur case if u dnt have a copy and neither ur vaccination history is complete i would recommend you to take necessary shots from the panel physician where ur live. Go with ur medical history record otherwise they will give u all vaccines. U have two options take risk file AOS if they need medical record they will give u RFE second option get ur medical done again where u live and file AOS.

They cannot go back to the panel physician in their country after their medical is already completed and get the vaccinations. The vaccinations must be done and marked off on the DS-3025 at the time of their medical. If you have any vaccinations after that, in your own country or in the US, you need to see a civil surgeon to have the vaccinations transcribed onto an I-693. You do not need to get a medical done again. The OP only needs the vaccinations. If they know for sure they had them all at the time of their medical, then they can skip the civil surgeon still. All their medical documents, including the DS-3025 will be in their file with the USCIS already. The worst that could happen is that they get an RFE for missing vaccinations and then need to deal with finding a civil surgeon that won't rip them off and insist on a whole new medical that is not required.
