Hi there, I am not sure what last name to put on the forms. Where it says family name, would I use my married name and use my maiden name where it says other names used? or do I just use my maiden name on all forms, since I have no identification with my married name on it. The I-693 has only my maiden name because the physician said that he had to use the name on my ID.

Any help will be greatly appreciated, Thank you.

There is no 693 form for a spousal visa.

the 693 is the examination and vaccination record, we're filing the I-130 for a Spouse Inside the US

I know what a 693 is, and again, it is not used for a spousal visa.

You posted in the spousal visa section and if you are inside the USA filing adjustment of status you are not filing for a spousal visa.

The 693 is only used with an I-130 if you are filing concurrent paper work to adjust status from within the USA. The 693 only goes with the I-485 not the I-130.

If your using your married name then you put your married name as your name.

It sounds like you're adjusting status inside the U.S.? Use your married name if you changed your name as a result of your marriage. Put your maiden name under other names used. Your marriage certificate is your "identification with your married name" and is sufficient proof of the name change.

I doubt that your case will get held up if your physician refuses to put your married name on the I-693.
