Form DS-3025 says my vaccine history is incomplete?

Hello everyone,

FYI: I have already read http://www.visajourney.com/forums/topic/101202-faqs-forms-i-693-and-ds-3025/ :thumbs:

I am working on our AOS file and have a question about my medical, which took place in London UK. I completed all the vaccination before my medical exam for K1 and on the day of the exam the Dr. said" well done you have all your vaccination done" :dance:

Now that I am looking at DS-3025 it says it incomplete? I think i need to take boosters not sure here really? I tried to call our local UCIS approved Dr and they are saying they will change $400 or more for the form I-693? Any one else who have been in my situation ? :help:

Any help will be great.

Mine was incomplete too, but all my vaccines were up to date... there were just some "not age appropriate", that's why the Dr checked it was incomplete.

Are you missing a vaccine or is it like me?

I only submitted a copy of my DS-3025 with my AOS package.

Did you take the booster shots for Measles-mumps-rubella on mine its says due in 1 month?

for influenza: due in 1 yr

TD: due in next 10 yrs.

I wanted to make sure before I send my AOS and get a REF for this. Any help will be greatly apprecaited.

To answer you, yes, I got all my shots, my next vaccine is due in 2018... Everything is fine on my DS-3025, just have some "not age appropriate" boxes checked. I'm not sure what you need to do as you need to have a shot done in 1 month, and it will take months before someone look at your AOS case.

Your vaccination record is incomplete, but is the "may be eligible for a blanket waiver" checked too? If yes, you have 2 choices: only send a copy of your DS-3025, maybe the adjudicator won't look into the details and won't see you need a shoot in one month... or maybe not and you will get a RFE (I may also get a RFE if the USCIS says they lost my medical files that were supposed to be in the sealed envelop I gave at POE). OR go to a civil surgeon and have the I-683 complete (after your booster shot, preferably).

Let's say only send the DS-3025 is more risky, but worst case you have a RFE, you go see a civil surgeon ($$) and your case is delayed for a few weeks. Or you take the safer route, don't want your case delayed and go see a civil surgeon now. :)
