A few questions about i-751, just to be sure I am doing it right:

1. Do former K-2s have A-numbers? There is a USCIS number on the child's card where A-number on the mother's, is that what I should put down?

2. Is it important to list middle names for "names used"?

3. For evidence--is it important to have everything listed on instructions? Our mortgage is in my name alone, but we have a child, joint bank account, joint tax returns, photos, phone bills.

4. Affidavits--mandatory, right? I get the impression that the chances of the person who signs being asked to testify are slim, right?

5. Fee: for a wife and child: $505+$85*2=675, correct?

6. The only place I see to list the child is in Section 5. I am not missing anything, am I? Anything I should do to make sure they catch that we are not just applying for the mother but for the son as well?

Affidavits are not mandatory nor are they needed if you have enough evidence of co-mingling lives and finances.

The A number should be on the Green card

2. Is it important to list middle names for "names used"?

I sure did for mine

3. For evidence--is it important to have everything listed on instructions? Our mortgage is in my name alone, but we have a child, joint bank account, joint tax returns, photos, phone bills.

You send what you have. If it is not too difficult I would try to add her to the mortgage. Otherwise, maybe include an explanation why it is only in your name. Some people send driver licenses or IDs showing same address

We didn't send affidavits. Lots of people don't include these. But I don't think they would hurt.

5. Fee: for a wife and child: $505+$85*2=675, correct?

Correct as long the child received conditional residency concurrently with mom.

6. The only place I see to list the child is in Section 5. I am not missing anything, am I? Anything I should do to make sure they catch that we are not just applying for the mother but for the son as well?

You can write in the very beginning of your cover letter that this application is to remove conditions on residency for your wife and son.

The USCIS number and A number are the same thing.

You don't need everything on the list. Our mortgage is in my husbands name only, and it wasn't an issue. As long as you have plenty of other evidence, you'll be fine without it.
