Hello!

I desperately need help with filling I-693 form. I don't know if I need a medical exam by a civil surgeon.

My DS- 3025 says: "Vaccine history incomplete" and "Applicant may be eligible for blanket waiver(s) because vaccination(s) not medically appropriate"

What does it mean?

Do I need another medical exam and more vaccinations? Or do I just have the DS- 3025 and the first page of the I-693 form for the AOS?

I really need help with this. I will be very grateful to whoever can help me out and provide that information.

You most likely do not need a I-693 because your vaccines are complete (eligible for blanket waiver). However, DO NOT send in I-693 by yourself. That is not a form for the applicant, that is a form for a Civil Surgeon. If you try to send in your own form, I guarantee they will ask for a I-693 filled out by a Civil Surgeon.

If you read through that post and do not need to see a civil surgeon, then just send in a copy of your DS-3025 with your AOS paperwork. As Harpa Timsah already mentioned, do not send in any portions of the I-693 what-so-ever. That form is only used when seeing a civil surgeon.
