Neeed some help here. When we sent our K1 package which is approved; we forgot to write in the section

where they asked about previous names used. Now i realised that my fiance' changed her first name because all her school and work documents have her current name on them.. she never really used the name that was originally on her birth certificate. can we just add that info on the packet #4 forms because

her birthcertificate states at the bottom that the first name was changed. Also, should we do a police clearance also with the oither name just in case? Really appreciate any and every suggestions. Thanks.

Me personally.. NO... You have an unusual situation and you need to be patient and allow members to wake up as you posted this in the middle of the night....

I have had a chance to think about this, how old was she when she changed her name (sine the notation was on he bc I assume it was a legal name change)? Is there an official document that was filed legally changing the name? My initial thought is if the name change occured before the age of 16 then a police certificate would not be required but that may not be your biggest hurdle.

I do not have any idea how to bridge the ommision on the original petition and wether it is a simple as adding it to packet 4.

I had this situation, i forgot to put my old last name on the petition. Before my mom got re marriead.

All i did was take proof of was who i said i was and school records, immunisation records, from when i was a child.

(1) OLD FIRST NAME WAS NEVER USED--ALWAYS USED CURRENT FIRST NAME EVEN THOUGH IT WAS NEVER OFICIALLY (LEGALLY) CHANGED UNTIL LAST AUGUST 2005.

(2) ALL SCHOOL RECORDS AND EMPLOYMENT RECORDS ARE IN CURRENT LEGALLY CHANGED NAME

(3) HAS NBI (POLICE CLEARNACE) FOR BOTH OLD FIRST NAME AND LEGALLY CHANGED FIRST NAME
