I am including my wife in this email so she gets familiar with the process as well.

1. In my home country, we used two last names (Father's and mother's) how should I fill out the forms?

2. Should we only include the last three visits to the USA or all of them?

3. What forms should be signed?

4. Applicants signed all forms or wife/husband should sign to?

5. Shall we include the case number on each sheet of paper?

6. What documents should be legalized in terms of degrees. My wife graduated in 2009 and legalized all documents, are those still current?

Timeline

fill out the two last names as they are

fill out all the visits, add a sheet of paper if there is little space but make sure you write the ITEM NUMBER to which you are answering on any additional sheet of paper.

make sure you fill out your case number on top write corner of all the papers you are submitting.

you need to fill out forms DS-230 and forms DSP-122. your wife fill out same forms to, meaning you guys will have two copies of same forms one by you and another by ur wife.

you will your papers and she will sign hers`

at this point you guys just have to fill out the schools attended and degrees obtain, it is at the interview that you guys will present all the originals of all documentation. just make sure they are translated to english if they are not.

Also print your names and case number on the back of the photos.
