Forms/Medical and Self harm

Hey guys,

I've won the lottery and have a specific question about the DS-122 and DS-230, and how this affects to the medical and the interview.

In the DS-230 stands: "Any false statement or concealment of a material fact may result in your permanent exclusion from the United States."

And then question 40a: "An alien who has a communicable disease of public health significance; who has failed to present documentation of

having received vaccinations in accordance with U.S. law; who has or has had a physical or mental disorder that poses

or is likely to pose a threat to the safety or welfare of the alien or others; or who is a drug abuser or addict."

I had OCD and have been eight weeks in a open psychatric station. Since 2009 I'm still in a outpatient treatment, but I'm almost fully recovered.That's not a threat for myself or anothers. But I did self harm several times because off a hard time with personal things, but this capture of my live ended and I don't do that again. Now I'm wondering if self harm is a threat to myself. I'd not consider myself as a threat for myself, my psychotherapist too. Otherwise I don't want to hide it, because it's an important fact.

I've read that my psychotherapist should write a letter which I have to give the doc in the Medical. But what should I do with the forms? Should I answer with YES or NO? Are there any same cases like mine? How do consulat officers see self harm?

I'm afraid that some stupid things in my life would destroy my dream.
