I have been reading post after post about forms I-485 and forms I-693.:( Some of the info says that I do not need to get a CS (Civil Surgeon) to fill in the form I-693 but can submit a copy of my DS-3025 form with the rest of my AOS forms. When I was in South Korea I had a TDAP vaccine and MMR vaccine injection given to me on the 13th February.I have Vericella history and on the DS-3025 form there is an x on results:vaccine history incomplete as well as an X saying:applicant may be eligible for blanket waiver because vaccination(s) not medically appropriate.

When I called a Civil Surgeon's office closest to my area(New Jersey,08060) they told me that in order to get my vaccinations transcribed,I would need to pay for a physical,tb test (they do not accept my t.b results which were copied on a c.d. at the Korean Hospital) as well as blood work for HIV and RTA.Does the USCIS rules from 2011 require that I have to get the form I-693 filled in and if I do not will my application be sent back requesting the I-693 be filled in???

yes, because your form DS-3025 is marked incomplete, and also marked you may be eligible for a blanket waiver. You need form I693 filled out. HOWEVER, you only needs parts 1, 2, and page 5 filled out. ONLY the vaccination record needs done. Call around your area for a CS explaining you only need Vaccination history filled out. It may take you some time to find a CS who will only do the vaccination record. If you cannot find a CS, go to your family physician, or primary care provider first. Get all lab tests done, and shots updated. Then take those results to the CS. I hope you find a CS who does this right.

my DS-3025 form was marked as incomplete because vaccination(s) was/were not medically appropriate. I did not have the I-693, just sent my ds-3025 and I was approved without a RFE. but...it's really a 50-50 chance. i've read about people getting a request for evidence because they didn't send form i-693, and others, just like me, got away only with sending ds-3025.

If you had the required vaccinations at the time of your medical and they are marked off on your DS-3025, then you do not need to see a CS and get the I-693, even if it is marked incomplete-may be eligible for blanket waiver. If you read through that post and decide you are in need of seeing a CS, then call around until you find one that won't insist on giving you another medical and/oe charging for it. You might need to call quite a few, and even look farther out from your area and drive a couple hours if need be to find one that won't rip you off.

Incorrect. Just because it is marked incomplete-may be eligible for a blanket waiver does not mean they need to see a CS. If they had all the required vaccinations for AOS at the time the DS-3025 was completed at their medical, then they can skip the CS. Also, if someone needs to see a CS, they would need PAGE 1, which includes parts 1, 2 and 3, and page 5, the vaccination record of the I-693.

he didn't have all his shots. we had a CS transcribe our vaccination and had it transcribed for about $45. they asked if we wanted/needed a physical and we said no. they told us that they could do the shots we needed there which would cost about $200 or so or we could go to our family doctor. he had to get varicella, mmr, and flu shot at our doctor then we brought back to the CS to transcribe. very straight forward and honest CS.

actually in talking with USCIS today, my wife's form was not marked so we were forced to do the I-693 with a civil surgeon, but I wanted to straighten out what I knew as well.

the DS-3025 form if vaccination lines Filled out and marked, as well as some marked "not age appropriate" is valid to send in. Just send in a copy, keep the original.

The "incomplete" means not all series of a vaccination are administered yet.

Reading between the lines of the conversation: You may or may not be subject to an RFE later on to check on the status of vaccinations. (This is accomplished by requesting an updated I-693) this is at the discretion of the adjudicator. Sometimes the length of time which has passed from the marriage to filing will trigger this audit.

HI guys, i'm on the same boat, about to submit all the AOS, EAD and AP forms. Got incomplete DS3025 (just page 1 of 2, right?)

The link Jay-Kay posted is very good, looking at the specifically ticks we need to NOT go to CS. Mine got Tdap (my doctor in Sydney gave me the shot on the day), Rubella, Measles, Mumps and Varicella all got dates. Rest are 'not age approp' and influenza is 'not fall season'. Strangely i showed him the proof for Hep B and Meningococcal but he didn't put them on it.

And just now i call him in Sydney, he said it's ok, he's done that plenty of times, but it's not because of the Tdap or x-ray. He said it's because the WHOLE set of vaccinations were not done all at his clinic, or something like that. He's a nice guy so I didn't ask more.

I'm a health worker and i need to have up-to-date vaccinations all the time, so i'm gonna write that somewhere in my documents, and submit more of my Hep B and meningococcal vaccine proof with DS-3025.

see how it goes. i will post the whole pack within this week.
