hi folks hope all is well

I'm trying to sent in my medical record from the psychologist does that has to be explicit ????please help i have no idea she ask me why to write ??? can anybody who ws approved help please please my lawyer hash t be helpful . this is my 7 session with the doctor .

The psychologist must write up a formal report stating the psychological problems that have resulted any medication and treatment. From what my attorney said this is a standard medical form and must be typed in this format.
