I765 questions 12 and 13

Hi everyone,

My fiance and I are filling out the form I765 and we are not sure what to put on questions 12 and 13 regarding date of last entry and place of last entry. She flew into Philadelphia and then we took a trip to Canada. Should be write down Philadelphia as the last place and date of entry? Or should I put the border crossing with Canada? Is there even any record of us crossing over into Canada? They didn't stamp her passport or anything.

You have no timeline. What type of visa did she enter on? You are calling her your fiancee and not wife in this post. If she came on a K-1, she was not allowed to leave the US and go to Canada before filing for AOS and having her AP or green card first. The I-765 would be filed with the I-485 paperwork for adjusting status after you after you are married.
