Ready to Send off i-485, but doing one last check...

hope everyone is doing well tonight^^

i-130 has been approved, so my husband and i are finally getting ready to send off my i-485, and all other necessary forms along with it.

however, i was hoping to post a few questions here, just to make sure that we've done everything properly, and have all the evidence we need for each corresponding form.

first off (this is probably a stupid question), in forms i-485, g-325a, i-765 and i-131, when it asks for my "Family Name" do they just want my current last name, or my maiden name? i'm assuming it's my current last name, since i don't sign anything with my maiden name anymore. again, i know that's probably a stupid question, but i just want to make sure everything has been filled out the right way.

second, the evidence we have for forms i-485, g-325a, i-765, i-131, and i-864 is as follows:

i-485 - a copy of the approved i-130 notice, copies of my passport identity page, i-94 slip, birth certificate, marriage certificate, 2 passport style photos.

i-765 - a copy of my passport identity page.

i-131 - a copy of my passport identity page, a copy of my approved i-130 notice, and a copy of my i-94.

i-864 - transcript of my husbands latest income tax return, 6 months worth of LES's (Leave and Earning Statement). my husband is in the army, so he doesn't actually receive pay stubs.

is that enough evidence for each form? are there things we're missing? or is there anything else we could add that might help?

third, is it absolutely necessary, in g-325a, where it asks "Last occupation abroad if not shown above", to list something there? the only other job that i can put there would leave a 10 month gap between the last job listed in the section above that question. i was told not to leave gaps in dates.

fourth, i was told to complete form i-131, to send along with my i-485, in case of an emergency. i'm a little confused as to how to fill it out though.

do i have to specify a date that i'm leaving and returning? cause i don't have any trips planned at the moment. should i leave Part 3 question 1 blank? should i put something like N/A in place of dates?

i think that's all i need help on, at the moment sorry for all the questions. i like to be thorough when doing paperwork

If that was his last occupation abroad, you need to list it regardless of the gap - the way to resolve that is by putting in the next line the gap dates and listing the occupation during that gap as "unemployed". If you need more lines because of that, just add an attachment that lists all the employment as well. I had to do that because I worked three jobs at one time.

For the I-131 you dont need any dates. So she's you can put n/a so they know it wasn't a mistake. They are used to it.

sorry for bumping this thread, but i have another question that i haven't been able to get answered through google.

when filling out the section in g-325a, that asks where you've lived for the last 5 years, what is the best way to write an apartment number?

Apt. 11? #11? 11-3200 Street Number?

is it necessary to scan the back side of my I-94 if there's no number on the back?

i know that's probably a really stupid question, but i see that mentioned a lot, so i just want to make sure that it's not needed if there's no nothing on the back.

Yes, you must copy the back of the I-94, even if it's just so they can see theres nothing written on the back. My husbands does have stuff written, so they probably want to see if there's stuff there or not.

And about the apartment number, just write it the way you see on the lease, or how the post office would normally address it. Theres no right or wrong answer for it. (or maybe how best fits in those teeny tiny boxes... XD)
