NO. But you can get the DS-3025 for free at the medical clinic in Kiev whereas the Civil Surgeon will cost you, possibly a LOT. Get all the vaccinations first (or at least started) take the records to the clinic and get the DS-3025 marked off as "vaccinations complete" and it will save you a lot of time and money

the DS-3025 is much easier and faster on the AOS forms as long as under one year, and marked complete, or filled in by the medical clinic. the pain is when marked incomplete, or not filled out. then you have to do the I-693 can be a pain to just get the section 1,2 and 5 filled out by a CS. You will have to call around in your area to find the right CS to fill out the form. Many CS want to perform a complete physical before filling out the form.

Best of luck on the Journey.... I miss Kiev and Odessa. They are wonderful cities. I hope to visit them again soon.

If you already had a CS fill out an I-693, then no, you do not need to include a copy of your DS-3025. There is a copy of the DS-3025 already in the file they have with the rest of your medical records.

The DS-3025 she received was not marked complete or incomplete. It was blank except for the doctors signature and date. Every doctor I called in my area insisted on another exam in order to complete the I-693 and yes it was expensive. Time and money seems to be the force that drives this process so why stop now. laughing.gif

I called USCIS and they told me that since Tatiana had her exam within a year all she needs is Part I and the vaccination record transfered to the 693. This is only going to cost me $10 at the local surgeon general.

A civil surgeon right? If she's not missing any shots she DOES NOT need an I-693 transcription

Yes civil surgeon, sorry. USCIS said told me that the I-693 with Part I and the vacination need to be sent in. I understand there is a lot of confusion if the 3025 will suffice. I am doing the 693 to be on the safe side. It's only $10 and possibly no RFE for that part.

As long as you get a CS to sign off on it hopefully you'll be okay. Some people tried sending it without a CS's signature and just part 1 and the DS-3025 which is wrong

The DS-3025 has to marked as complete. Otherwise you have to get he I-693 done for PARt 1, part2 ,and part 5. However, the problem is most CS won't even BOTHER just doing those parts. THE CS wants to perform a FULL medical exam. If all they do is the transcibe service MOST CS would go out of business since the exam with vaccinations is a money cash flow for them. No insurance to bother with. No worries if they are medically insured. The CS just has to meet the requirements to be CS which really ain't much. It is by far easier to be a CS then to have a thriving private practice. Most of the CS in Phoenix are not USC doctors. ( they may be USC now, but some names make you wonder if they are first generation or second generation immigrants themselves.)

Sad to say this is another part of the process which is a "racket" the K-1 visa does not need much. However, the CS does not spend time knowing exactly WHAT to do. They just say, I have to fill out this form completely so you gotta pay.

I took in the medical records, lab tests, vaccination history. All he had to do was say when and where. Nothing on his part but one shot and one TB skin test. COST ME out of packet: $245 plus 3 trips to the office, gas, time off work, and having to submit the claim myself. then the CS does not practice proper privacy rights. Exam rooms do not have floor to ceiling walls, so it is possible he has cams watching each room and receptionist does not even practice patient confidentiality properly. If I get and RFE for the form, definitely not going to him. The office reminds me of something out of 1950 to 1960 not something of the present doctor's offices. I kept on hoping Micheal J. Fox would pop out saying he was making Doc Hollywood II.

Sounds like you went to the same place I did. laughing.gif Seems the only reason this doc is in business is to do these exams in order to transfer the DS-3025 to the I-693 and mark it complete. I called at least 10 CS's here and none of them would just transfer the info without an exam. He was the cheapest at $310. When I reviewed the I-693 his receptionist filled in I found 3 errors. Some "racket" as you said.

It sounds like the DS-3025 was not filled out properly, you might try emailing the clinic in Kiev, clinicanafta@rambler.ru or iomkievmhd@iom.int to try and get it corrected, if you can't, CS are a ####### shoot as far as price and what they will do. The local CS wanted $300.00 to do the vaccination portion of the I-693 (page 1 & 5). I found one 250 miles away that charged me $40.00 ($20.00 each- I have a stepdaughter). You might be able to find one in your area here: http://www.visajourney.com/forums/topic/231026-civil-surgeons-who-dont-rip-you-off/, if you can't-- CALL AROUND, it may save you a lot of money :)

Exactly. It is no longer part 1, 2 and 5. The new form and instructions that must be used since Jan. 2012 require Page 1 and Page 5 to be completed by the CS. Because of this, it is a bit confusing as part 2 on Page 1 asks for a summary of your medical. A lot of the CS are insisting on a new medical because they cannot verify a medical you had overseas that they did not personally perform. It can be difficult and time consuming to find a CS that will still charge for only the transcription and not insist on a medical you are not required to receive. However, as you discovered, there are still some of them out there that do it correctly and don't intentionally rip you off.

There needs to be changes for those adjusting from a K-1 to make it more clear. If the instructions all state another medical is not required if you had yours done overseas within the last year, then a CS should not be able to insist those USCIS instructions are incorrect and you still MUST get the exam from them again. What is the point of stating you are not required to get it, if they allow the civil surgeons to force it upon people anyway?

CALL AROUND is the best advice. It may take awhile and get you very frustrated. You may have to drive out of your way to another town (we drove 2 hours away), but you can probably find one that still knows how to deal with the I-693 for a K-1 and not rob your wallet and give unnecessary tests.
