When we filled up the forms for the AOS(affidavit, etc), my wife has not yet acquired my last name. (my wife is my petitioner by the way). will this cause any issues if my wife did not use my last name when she completed the i-864 affidavit of support and some of the forms that needed her names?

she said because at time of application AOS, she has not formally filed a request for her last name to be changed as mines? er any issues we may encounter during AOS processing?

Formal request? Did she change her name when you two got married on the marriage license? If so, THAT is as formal as it gets for documentation. She can take a copy of the marriage license to SSI, DMV or whatever for changing her name on those cards and for anything else as for that matter. The marriage license is accepted for changing last names during marriage.

so this means that if we fill up the i-864 form (affidavit of support), we should have used her "new last name" then?

If you used a new last name on the marriage license... YES you should have put that name down on all forms.

just to check with you about the fee of the AOS. It will be a total of 1070 USD inclusive of the i-765(EAD) AND i-131(AP)?
