i-693, is it needed for AOS?

As for my case, during my medical at St Luke's,I already had 2 vaccine shots (TDAP and MMR) and we are filing the AOS within this same year. Does it mean I no longer need the i-693 right?

However for precautionary purposes and medical reasons, do I still need booster vaccine shots, optional though right?

and not for immigration purpose?

My fiancee is due one more shot when we get to the states also. The I-693 is still needed.

I don't know, I never looked at her paperwork from her medical and she did not say which shot, just said she was told she will need one more shot once we are home.

If the DS form from St. Lukes is filled out AND you are under 1 year. you can file with just the DS form. (make a copy)

If the DS-form is not filled out and it is under 1 year since St. Luke's you can go to CS and have them fill out ONLY the vaccination portion of the I-693 and submit that in a sealed envelope. Call around to find a CS who will do this. Many CS will want to perform the whole exam.

If it has been more then a year since the exam at St. luke's, the complete I-693 must be filled out.

I tried to find CS in Phoenix who would only do the vaccination records with no success. Every one wanted to perform a complete exam. Be diligent when looking and calling. Also, have the instruction pages to prove EXACTLY what you need for the AOS to show the CS. (This might have helped me) Also, the CS usually wants cash. Get a receipt for the exam and file with your Medical insurance. Also, any shots needed, have your PCP do it under the insurnace plan, then go to the CS. the CS will ask for cash on every shot as well. the reason is for the form. Once the form is fileld out and sealed to the USCIS, the CS usually do not get the rest of their money. Best of luck on this part of the road ahead.

02/11/2011 - Engaged at her house by her Godmother.

02/18/2011 - Engagement party with relatives - propose in Visayan.

02/24/2011 - K-1 packet sent.

09/18/2011 - POE, Viva Las Vegas, Baby !!!!! Home to Phoenix.

12/10/2011 - Official Wedding

07/05/2012 - Princess Rose born.

07/07/2012 - AP/EAD received.

07/17/2012 - AOS passed. (Birthday for Mama Rayos)

It's really hard to tell. IMO based on experience, I would say it really depends on the immigration officer . When I had my AOS interview they asked for my I-693. But it wasn't the same with my friend, in her case the immigration officer said it was okay that she didn't have the I-693. We had the same place of the interview both filed for the AOS before the I-94 expired and had the same shots that we needed to complete once we get here like the HPV vaccine, only that I was a year earlier.

Some people just do it so that there will be no more hassle in delaying the AOS approval, but some people prefer to wait until their interview or RFE to get the I-693
