I got some information that on April 2012, that form has been updated, unfortunately I did not find any template.

They mention it is optional but I would like to confirm that. If someone can assist me I really will appreciate it.

DS-2001has someone already sent by mail the notification that he/sheis prepared for the interview?(As per the changes the NVC has made onApril 2012)Do you have a template to be sent by email?thanks!
