Incorrect, the beneficiary's name (your wife) goes here, she is the applicant, not you. You are the petitioner.

Incorrect, the mailing address being asked for is the beneficiary's (i.e. your wife's mailing address).

5) Date of Marriage (if applicable): the date when you guys are married

Incorrect, the phone number being asked for is the beneficiary's (i.e. your wife's phone number).
