You do not need a full medical again, but you need to see a civil surgeon to get the shots you need and have them fill out the I-693 that you need for AOS. A lot will try and tell you that a new medical is required and charge you for it. Try calling around until you can find someone willing to do the vaccinations for you without insisting on the new medical and overcharging you. You might have to call around several places, and even check civil surgeons that are a longer driving distance away. You can print out the I-485 and I-693 instructions to prove you do not need a whole new medical, just the vaccinations.

Vaccinations are not a requirement for them to issue the K-1 Visa however, as you have found out, it is a requirement for AOS.

It seems to me that you will be required to get all the vaccinations that you don't have proof of getting previously. I did a lot of research on this subject in the past and found this website very helpful to determine exactly what is required. http://www.immunize.org/catg.d/p4030.pdf

As stated by another poster, you will want to go to a civil surgeon who is on the approved list for your area because they will need to fill out the I-169 to document that you have received the required vaccinations.

Depending on when your medical was done (if it hasn't been longer than a year ago) then you should not be required to get the full medical, only the vaccinations.

Good Luck!
