OK, so I'm starting to fill out the AOS forms.

Is the I-693 form in the sealed packet she got after the medical? Or do I have to take her to a civil surgeon after she gets here?

Thanks.

Will have to take her to a civil surgeon, but you should bring her vaccination records and any other health records she has to the surgeon to minimize things the surgeon has to do to fill it in.

You can download the I-693 from USCIS site

http://www.uscis.gov/portal/site/uscis/menuitem.5af9bb95919f35e66f614176543f6d1a/?vgnextoid=eb1f3591ec04d010VgnVCM10000048f3d6a1RCRD&vgnextchannel=dd346d26d17df110VgnVCM1000004718190aRCRD

After she gets here, you will take the Immunization Record (Yellow form), Vaccination Document Worksheet (DS-3025) she has. They are not in the sealed package...bring to the Civil Surgeon to have he/she fills it in for you.

You will fill out Part 1 and the CS will fill out Parts 2 & 5 on the I-693. After the exam, the CS will then sign and seal it for you to submit to USCIS. Remember Vaccination Only..if you Med is current at that time.

Thanks for that. Great info. I made an appointment with a Civil Surgeon for the day after she arrives.

I think you should get married before you start going through the motions for adjustment of status. The civil surgeon will only take an hour or two. If you decide to do the full medical then it will only take 3 days. No need to rush this.
