NVC Documents I-864? anyone?

ok so i am at the NVC stage. now i have to complete the Affidavit of Support.

i understand it is 125% of poverty line. i only made 12,000 in 2011.

so my dad who lives in the same house as i do will be helping me out since

he made about 40,000 and has 3 dependents.

what I-864 forms do we have to fill out? i-864a? i-864ez ect??

so i know i have to fill out a i-864

and my dad has to fill out i-864a? or i-864ez?

also he is self employed so is there anything else he must

do?

You both fill out an I-864. And you both include tax information (either tax transcripts or tax returns, W2s, and all schedules). Your dad will also have to include proof of his US citizenship/residency. Also including a copy of your birth certificate (to prove he's your father) would be recommended.
