Hello guys!

I got a question. Hope someone out there can help me to make everything clear. I will be extremely thankful. :)

On Apr. 10 Im coming to the USA on K1 visa. After get married I will apply for Adjustment of Status but even right now I see I will get a problem with my medical exam for AOS. When I was passing the medical exam in Kyiv (UKRAINE) I didnt bring with me the vaccine report, so the I685, or DS 3025 is going to be incomplete. I cant say what exactly and cant see because all the documents are in the sealed brown package which I cant open.

Somewhere it was said that i have to bring with myself the vaccine report which is issued in my hospital, translated and notarized to the USA. Then I have to search for the civil surgeon who is working with USCIS in my area, and bring this report to him, and he will complete everything what is necessary (all the vaccine fields). Am I right about that? Can I be sure that this vaccine report is enough? Just want to be prepared. ps. moreover my vaccine report is written by hand by my doctor, signed by him and the head doctor, and it has all the seals for medical documents. Is it fine?
