does anyone know the exact address of the Philippine embassy in Dallas Texas? because I need to change the my last name in my passport...

The embassy is in Washington DC and there doesn't look like there is a consulate in Dallas.

All foreign embassies are in Wash D.C. You may want to check their website for locations within the US of their consulor offices and which would have jurisdiction over Dallas.

these are the requirements:

Accomplished Report of Marriage forms (in triplicate)

Certified Copy of the Marriage Certificate (original and three [3] copies)

NSO Certified Birth Certificate of the female applicant (original and three [3] copies)

Three (3) copies of photo identification of both husband and wife

NSO Certified Death certificate of previous husband (if needed) (original and three [3] copies)
