quick question - i am filing for AOS (i did my POE from canada on March 4th 2012)...do I need to get form i-693 filled out? or do i just include the worksheet with my vaccination records from my medical?? please let me know!! Thanks!

There's alot of people who would tell you its fine to send it as it is, and there's some who had and RFE for the DS-3025 not being complete. Here's some links so you can determine if you need it or not. Good Luck

I would rather spend $50 to $80 and get them transcribed to i-693, to avoid any threat of medical RFE altogether and possibly slowing down of process.

Its each one to their own, peace of mind is more important for me but depends on if someone is willing to take a chance with only DS-3025

Most of us in Canada who had our medical at teh same place as you did'nt need to have anything transcribed. The paper you got the "medical worksheet two" is the equivalent of the i-693 vaccination sheet. you should have received exra copies of that as well as your sealed packet. Just send that in.
