Hi, From what I have read here on VJ, If the DS-3025 is checked "Complete" you can include it with your AOS package. We will be including both the DS-3025 & I-693. If your DS-3025 is marked "incomplete" you should make an appointment with a Civil Surgeon so he can complete what is needed on the I-693. Relationship evidence is taken with you at the time of your interview. Good Luck :)

If you had all the required vaccinations at the time of your medical, even if your DS-3025 is marked 'incomplete- may be eligible for blanket waiver', you can skip the civil surgeon and just send a copy of your DS-3025 with your AOS documents. If you still need vaccinations, or had any done after your medical, then you need to see a civil surgeon to get an I-693.
